Financial power of attorney. Names an agent (attorney-in-fact) to act for you on money, property or business. LawDepot sets out the types: a durable power stays valid if you become incapacitated, an ordinary one ends on incapacity, and a springing one takes effect only after a triggering event such as a date or incapacity. It also notes that this document does not cover medical decisions1.
Health care directive (living will). Records which treatments you want or refuse and can name an agent for medical decisions. The California form shown on Rocket Lawyer's living will page says it must be signed by two qualified witnesses or acknowledged before a notary public2. Other states set their own signing rules.
